Why Your Lateral Shoulder Still Hurts After Rehab — And the One Exercise That Actually Fixes It

Why PT Often Falls Short (It’s Not the Therapist’s Fault)

That stabilizer is your Serratus Anterior (SA) — the deep rotator cuff muscle that controls your shoulder blade.

When that happens, the deltoid takes over, overworks, and creates the exact lateral arm pain you feel when you reach, lift, or sleep.

So here’s the fix — and it starts working fast.

The Serratus Reset: Forearm Wall Slide + Reach

Do this gently. This is a reactivation, not a workout.

Step 1 — Set Up

Stand a foot from a wall. Place your forearms against the wall, elbows at shoulder height.

Step 2 — Slide

Slowly slide your forearms upward, only as far as you can WITHOUT pain.

Step 3 — Reach (The Magic Part)

Pause. Now gently push your forearms into the wall, like you’re lengthening your arms upward.

You should feel your shoulder blades glide and wrap slightly around your ribs. That’s your serratus waking up.

Step 4 — Breathe

Take a slow 4–6 second exhale.This shuts off the tension and allows the SA to fire.

Step 5 — Repeat

5–6 reps, 2–3 times per day.

Most people feel:

  • Less deltoid burning
  • Less pinching
  • More elevation
  • Calmer pain when reaching

That’s the shoulder finally moving the way it’s supposed to.
